Pages 1-2 are instructions while page 3 is the form you need to enclose with your completed fingerprint cards and certified funds (you must submit certified funds or it will be returned unprocessed). The remaining pages include information about the AOC Fastcheck alternate program Kentucky residents may be able to use temporarily while waiting on the FBI report to be returned.
